Financial Information

A facility fee will be charged for services rendered by Parham Surgery Center. The facility fee is separate from any fees charged by your doctor, pathologist, and anesthesiologist.

 Insurance Accounts - In order to determine you benefit coverage a completed registration form is required as soon as possible after you are scheduled for surgery.  Please have information available about your insurance coverage or other methods of payment.

Self-Pay accounts If your procedure is not being covered by insurance, you may request an estimated facility fee at 285-4763. The estimate is based on CPT codes provided to us by your physician.  If you do not have insurance benefits, a deposit of fifty percent of the estimated charges will be due on the date of surgery and payment arrangements may be set up for the balance.

When verifying your insurance coverage for your procedure(s) you will need to tell the insurance company that Parham Surgery Center is a freestanding ambulatory surgery center.

The day of surgery you will be given surgical, anesthesia and financial forms to be signed before your procedure.

Accreditation and Regulation
Medicare and the Accreditation Association for Ambulatory Health Care (AAAHC) accredit Parham Surgery Center.
